You will need:
- I.D. of Guardian
- Proof of Address
- Social Security Card and Birth Certificate
- Food stamp and Medicaid cards (if the student receives those benefits)
- Immunization Records & 3300 form (after the student turns four years of age)
Students must be four years old before September 1st.
